Our Mission

Our mission is to bridge gaps in care by providing compassionate, coordinated support through discharge planning, case management, and therapy services—empowering individuals, couples, and families to navigate life transitions with clarity, dignity, and hope.

Our Vision

We envision a world where no one falls through the cracks of the healthcare or mental health system, and where every person has access to continuous, personalized support that fosters healing, stability, and long-term well-being.

Our Founder

Fatme Ancouni, LMSW

I am a Licensed Master Social Worker (LMSW) with over 10 years of experience supporting people through life’s challenges. I’ve worked as a mental health therapist and independent contractor in a variety of settings, helping children, adults, seniors, and individuals with developmental disabilities feel heard, supported, and empowered. I have a special passion for working with older adults and individuals living with trauma and PTSD, and I use a trauma-informed, compassionate approach that meets people where they are. I love this work because I believe in people’s ability to heal, grow, and find meaning even after difficult experiences. It is important to me to create a safe, welcoming space built on trust, patience, and genuine connection, where clients can move forward at their own pace and feel respected every step of the way.
